Laboratory Test Order Forms

Specimen collection and handling: We want to provide the highest quality lab results as quickly as possible. That’s why we need to ensure that all samples are collected and handled in a way that is compatible with validated standards and testing procedures. Please ensure that each specimen submitted for testing is fully labeled and the accompanying requisition is completed in full. Improperly labeled specimens or incomplete requisitions may result in delayed testing or specimen rejection.

 

Requests or samples may be rejected for the following reasons: Incorrect specimen type, insufficient volume, grossly hemolyzed specimen, damaged or leaking specimen container, low viral load if appropriate (see requisition), incorrect or missing labeling or requisition, or delayed or improper transport.

 

Specimen should be packaged as per instructions on the provided requisition and shipped according to IATA and TDG guidelines (UN 3373, Category B). If specimens are being sent directly to the BC-CfE laboratory, please notify lab in advance by email (lab_clerks@bccfe.ca) with the tracking information.
