The Psychologist, CFE HIV/AIDS reports to the Professional Practice Leader, Psychology, Providence Health Care and works under the clinical direction of the Regional Practice Leader – Psychology Professional Practice, Vancouver Coastal Health. The Psychologist, CFE HIV/AIDS is responsible for the provision of a full range psychological, supervisory, and program evaluation services.Working independently and as a member of an interdisciplinary team, completes psychological assessments, provides psychological treatment, evaluates the effectiveness and appropriateness of treatment and facilitates education of clients. Maintains client records including the documentation of client diagnosis and most effective form of treatment and prepares progress notes and psychological reports. Liaises with external agencies and establishes and maintains effective working relationships with stakeholders. Attends clinical/case conferences. Provides formal and informal consultations, participates in quality improvement and risk management activities, provides clinical teaching and supervision to students and conducts and participates in research.

The Family Physician is a core member of each interdisciplinary clinical team that shares responsibility for the care of a panel of clients. Interdisciplinary teams are comprised of the Family Physicians, Nurses, Social Workers, Medical Office Assistants and Peer Support Specialists. They are supported by shared access to other health providers, such as psychologists, registered dietitians, physiotherapists, clinical pharmacists, phlebotomists, vocational therapists, and occupational therapists. The clinic is supported by two community psychiatrists who manage and follow patients concurrently with the primary care team.

Hope to Health primary care clinic is recruiting for Nurse Practitioners who will be part of a supportive multi-disciplinary care team that includes an RN, LPN , social worker, peer support worker, and MOAs, as well as in house access to other health providers: Family physicians, PT, OT, dietitian, psychologist, clinical pharmacist, phlebotomist and psychiatry consulting services.

Reporting to the Clinical Operations Manager and working in collaboration with the interdisciplinary team, the duties of Registered Nurse/ Registered Psychiatric Nurse (RPN) include working with the health care team to assess and engage new clients/patients, documenting assessments and developing and carrying out care plans and interventions in consultation with team members. In collaboration with team members, the Primary Care Nurse participates and facilitates team and case management meetings.

Reporting to the Clinical Operations Manager and working in collaboration with the interdisciplinary team, the Licensed Practical Nurse is responsible for providing nursing care to an assigned group of patients/clients according to established policies and procedures and the Standards of Practice for the Licensed Practical Nurses of British Columbia. Duties include participating in care conferences and in developing care plans, conducting assessments, implementing and evaluating interventions, documenting information, and acting as an advocate for patients/residents.
