The Program Medical Director (“PMD”), Hope to Health Research and Innovation Complex (H2H), is responsible, in partnership with the Clinical Operations Director, for the planning and implementation of existing (harm reduction, safer drug supply, supervised consumption site and primary care) and future clinical services at H2H. Hope to Health primary care clinic is recruiting for Nurse Practitioners who will be part of a supportive multi-disciplinary care team that includes an RN, LPN , social worker, peer support worker, and MOAs, as well as in house access to other health providers: Family physicians, PT, OT, dietitian, psychologist, clinical pharmacist, phlebotomist and psychiatry consulting services. Family Physician

The Family Physician is a core member of each interdisciplinary clinical team that shares responsibility for the care of a panel of clients. Interdisciplinary teams are comprised of the Family Physicians, Nurses, Social Workers, Medical Office Assistants and Peer Support Specialists.  They are supported by shared access to other health providers, such as psychologists, registered dietitians, physiotherapists, clinical pharmacists, phlebotomists, and occupational therapists. Specialist physicians are also an on-site and shared support for the clinical teams to refer to and meet the needs of their clients, including psychiatrists.  The current operating hours are between 8:30 AM and 4:30PM Monday to Friday.

Reporting to the Clinical Operations Manager, and receiving professional practice leadership and support from the Professional Practice Leader, Physiotherapy, this position supports the interdisciplinary Hope to Health Team (which also includes social workers, nursing, psychologists, physicians, pharmacists, etc.) in achieving program and patient goals. This includes providing evidence based clinical assessment, treatment and education grounded in physiotherapy theory and relevant post-graduate studies to outpatients with movement dysfunction, musculoskeletal and chronic/complex pain issues within a group setting and/or on an individual basis. Provides leadership and expertise in the evaluation and treatment of physical functioning/abilities, provides a therapeutic environment and self-management education, monitors patient outcomes and adjusts care plans as required by working collaboratively with patients, families and caregivers, and outside agencies.

Peer Support Specialist

Reporting to the Clinical Operations Manager, the Peer Support Specialist (PSS) shares their own consumer knowledge and lived experience of mental illness / substance use and experience navigating clinical services to assist clients to reduce barriers to care and support engagement and attachment. The PSS collaborates with the client and the clinic health care team to determine appropriate care to meet individual needs and situational requirements. The PSS provides clients with education, goal setting, support and advocacy from a lived experience perspective in order to assist clients in effectively obtaining service from the ICT team staff, external service providers and community resources. Provides a variety of supports to assist clients living with mental illness / substance use such as emotional and social supports, health care navigation, goal setting, information, resources, and demonstrations. The PSS participates in care planning, team meetings, quality initiatives and continuing education by sharing and discussing information, presenting material and suggesting changes to policies and procedure. The PSS brings the voice of lived experience to conversations about clients, programs and systems. The PSS has a willingness to be identified as a peer in the areas of mental health and/or substance use and/or homelessness and the ability to use that recovery experience and recovery principles as a practice.
