Primary Care Clinic

For many people, particularly in Vancouver’s Downtown Eastside (DTES), who face complex health, mental-health and addiction challenges, access to primary care can be difficult and require low-barrier services. Designed to provide a broad range of accessible psychosocial and primary care services, the Primary Care Clinic (PCC) at H2H is interdisciplinary, focusing on a client’s needs and offering a coordinated and proactive care approach. Serving clients with complex health care needs, unattached to primary care, the PCC provides essential care for those living or receiving most of their services in the DTES of Vancouver.

 

With the goal of enrolling, engaging and empowering these clients to improve their clinical outcomes, the services are akin to “ambulatory critical care” for a priority population. Close monitoring of the clients served by H2H with real-time analytics and a solid quality improvement approach, has been key to its success.

 

Home to over 1,800 clients, recent funding announced by the provincial government will provide resources to expand this capacity to 2,800 total clients over the next year. Taking a holistic approach to healthcare delivery, teams of specialists work together to provide comprehensive, patient-centred care to help these clients meet their healthcare goals.

Hope to Health Staffing

H2H staffing consists of various primary care teams and one triage team. Each primary care team has a family doctor, social worker, peer, medical office assistant, and casual LPN and RNs. Teams are supported by a behavioural consultant, a clinical pharmacist, a phlebotomist and a clinic coordinator. The triage team focuses on triaging walk-ins, and unattached clients who may have urgent care needs. Limited psychiatric consultations are provided by an on-site psychiatrist. Other specialized services are being offered including Infectious Diseases Consultations, behavioural interventions to manage Stimulant Use Disorders and Complex Chronic Pain and substance use disorders.

Hope to Health Services

H2H is open during regular business hours with access for after-hours consultation by an on-call physician. Services are wrapped around client needs, with proactive system of care, and use of Electronic Medical Record (EMR).

Hope to Health Performance and Quality

H2H clinic service uses performance and quality measures to drive quality improvement. A scientific evaluation of H2H will be carried out with a grant from the Canadian Institute of Health Research (CIHR).

During the Canada Post strike, the following measures have been undertaken to minimize service disruption to BC-CfE clients and providers.

  • The BC-CfE Drug Treatment Program (DTP) will fax outgoing forms and documents to the provider’s office.  (DTP Contact Information: Phone 604-806-8515; FAX 604-806-9044)
  • St. Paul’s Hospital Ambulatory Pharmacy has transitioned to private courier for delivery of medications. (Contact Information: Phone 1-800-547-3622; FAX 604-806-8675)

During this time, we recommend that documents be faxed or couriered to our sites, versus utilization of regular mail service.
