Hope to Health Primary Care Clinic

Reporting to the Clinical Operations Manager and receiving overall professional guidance from the Professional Practice Leader, Social Work, the position supports the interdisciplinary team in achieving program and patient/client goals. Duties include providing therapeutic clinical counselling services, conducting comprehensive biopsychosocial assessments and maintaining patient/client records. Provides consultative and direct services to patients/clients, staff and health professionals regarding community resources. Coordinates clinical/case conferences, develops and delivers presentation for staff and maintains broad knowledge of development in the Social Work profession.

Reporting to the Clinical Operations Manager and working in collaboration with the interdisciplinary team, the Licensed Practical Nurse (LPN) is responsible for providing nursing care to an assigned group of patients/clients according to established policies and procedures and the Standards of Practice for the Licensed Practical Nurses of British Columbia. Duties include participating in care conferences and in developing care plans, conducting assessments, implementing and evaluating interventions, documenting information, and acting as an advocate for patients/residents.

Reporting to Clinical Operations Manager, the Medical Office Assistant acts as a member of the client care team and performs a variety of clerical duties utilizing computerized software programs in support of the operation of the unit.  Acts as a communication link for the unit, providing receptionist functions, coordinating client appointments, and assembling and maintaining client charts.

The duties of Registered Nurse/ Registered Psychiatric Nurse (RPN) include working with the health care team to assess and engage new clients/patients, documenting assessments and developing and carrying out care plans and interventions in consultation with team members.  In collaboration with team members, the Primary Care Nurse participates and facilitates team and case management meetings
