Hope to Health Primary Care Clinic, a program of the BC Centre for Excellence (BC-CfE), is expanding its interdisciplinary care team. Located in Vancouver’s Downtown Eastside, the clinic provides a team-based primary care with both scheduled and drop-in services for clients with complex medical and psychosocial conditions. The clinic has 16 exam rooms and 6 consult rooms, there is also a shared area where physicians/nurse practitioners, other clinicians and support staff work collaboratively within an integrated care environment.
Hope to Health offers a wide range of interdisciplinary primary care services and social supports including:
- Mental Health Support
- Addiction treatment and harm reduction services
- HIV and Hepatitis C treatment and prevention
- General primary and preventative health care for adults

The clinic is quality improvement-oriented and provides opportunities for staff to participate in ongoing clinical research in collaboration with BC-CfE’s experienced leadership and research teams.Â

Job Specifics
The Nurse Practitioner (NP) will be an integral member of the Hope to Health interdisciplinary care team, working collaboratively with physicians, nurses, social workers, peers, and other allied health professionals to deliver comprehensive, person-centered primary care.
The NP will establish and manage their own patient panel, while also contributing to the clinic’s shared care model and supporting clients with complex and diverse health and social needs.
The NP may be required to participate on the on-call roster. Weekly afterhours calls are from Monday to Monday from 5 pm to 8am on week days and full day on weekends. Currently, the call schedule is one week every 7-8 weeks and are equally distributed between MD’s .
This role is part of Vancouver’s Primary Care Networks (PCN) initiative and offers an opportunity to participate in shaping integrated primary care delivery within the community. This position can be full-time or part-time and is salaried.  There is protected time for training and QI activities, and for indirect care.
Key Responsibilities
- Provide comprehensive primary care assessment, diagnosis, and treatment within NP scope of practice.
- Collaborate with interdisciplinary team members to coordinate client care and facilitate referrals as needed.
- Manage a patient panel and maintain appropriate clinical documentation and reporting in accordance with PCN and BC-CfE standards.
- Support harm reduction and trauma-informed practices across the care continuum.
- Participate in clinical quality improvement initiatives and research activities as appropriate.
- Contribute to ongoing development of the Hope to Health team-based model and integration with community PCN partners.
