STATUS:  Part-Time to Full-Time work is Available (Minimum 0.4 FTE, can accommodate up to 1.0 FTE as desired)  

JOB START DATE: As soon as possible

SALARY: $207.77/hr ($349,048/annum for 1.0 FTE)- Based on CSC GP – FULL SCOPE (NON-JSC COMMUNITY)

LOCATION: Hope to Health Clinic, 625 Powell Street, Vancouver, BC

ORGANIZATION:

The Hope to Health (H2H) Primary Care Clinic uses a low-barrier, team-based care model to deliver client centered and proactive longitudinal primary care and substance use treatment for clients that reside or receive the majority of their care in the Vancouver Downtown East-side.

The H2H clinic works with clients who have complex medical or social needs, to prevent and/or improve outcomes related to HIV, Hep C, mental illness and substance use and other chronic conditions. The clinic offers OAT as well as iOAT.

The H2H Research and Innovation Centre is affiliated to the BC-Centre for Excellence in HIV/AIDS (BC-CfE) and Providence Health Care (PHC). The BC-CfE is a world-renowned HIV/AIDS research centre with an integrated group of research concentrations in Laboratory Sciences, Clinical Trials, Population Health and Epidemiology, Health Economics and Professional Education Programs.

JOB:

The Family Physician is a core member of each interdisciplinary clinical team that shares responsibility for the care of a panel of clients. Interdisciplinary teams are comprised of the Family Physicians, Nurses, Social Workers, Medical Office Assistants and Peer Support Specialists.  They are supported by shared access to other health providers, such as psychologists, registered dietitians, physiotherapists, clinical pharmacists, phlebotomists, vocational therapists, and occupational therapists. The clinic is supported by two community psychiatrists who manage and follow patients concurrently with the primary care team.  The current operating hours are between 8:30 AM and 4:30PM Monday to Friday.  

The interdisciplinary clinical team work at the H2H primary care clinic, is enhanced by ongoing monitoring, evaluation, quality improvement as well as clinical and epidemiological research and knowledge translation.

The H2H complex includes harm reduction programs, an Outpatient Parenteral Therapy unit (OPT)and on-site pharmacy. The H2H complex is affiliated with the 611 Safe Consumption Site which provides injecting booths and smoking inhalation rooms.
The H2H clinical primary care teams continue to expand and we are seeking family physicians to join our growing interdisciplinary teams to do part-time or full-time duties (ideally 4 to 10 half days a week).

The H2H primary care clinic does not provide Prenatal, Postnatal, Neonatal, or Pediatric (18 and under, except in exceptional circumstances) care.

JOB RESPONSIBILITIES:

Clinical Services:
All clinical activities associated with the medical practice of a Family Practitioner providing outpatient care including:

  • Continuing care through active assessment and treatment
  • Treatment planning, surveillance, and follow-up.
  • Participation in interdisciplinary team care conferencing and medical coordination including, disengagement assessment and complex care planning.
  • Provision of OAT and iOAT
  • Documentation, medical orders, referrals and care coordination with other programs, providers and team members.
  • Shared on-call duties, schedule 1-week at time and occurs every 7 to 8 weeks depending on number of providers in the clinic (compensated at MOCAP rates)

Clinically related teaching and clinically-related research:

  • The H2H Primary Care Clinic is a training site with clinically-related teaching of trainees, medical students, medical residents, clinical fellows and other health professionals occurring concurrently with the direct and indirect care.  Additionally, clinically-related research concerning the needs of a particular client (for example, investigating the client specific appropriateness of particular diagnostic and therapeutic interventions)

Clinical administrative services:

  • Other work requiring the professional expertise of a physician that may not be client-specific, such as participation in quality improvement or quality assurance activities, audits, and reviews, educational rounds, attendance at clinical meetings/committees and program planning and development activities.

JOB QUALIFICATIONS:

Education, Training and Experience

  • Current College of Physicians of BC (CPSBC) license – Full – in good standing
  • Canadian College of Family Practitioners (CCFP) is an asset
  • Canadian Medical Protective Association (CMPA) – Mandatory
  • A strong preference will be given to those with expert knowledge of inner-city primary care and substance use disorders.
  • Ability to work alongside and in partnership with colleagues to help the Primary Care team achieve its aims and objectives.
  • Medical Staff Privileges at Providence Health (current, or will be obtained)
  • Appointment with the UBC department of Family Practice an asset.
  • Completed the BCCSU POATSP training and able to prescribe OAT and iOAT (candidates can supported to learn iOAT prescribing if not currently certified).

The BC-CfE Laboratory is streamlining reporting processes for certain tests in order to simplify distribution and record-keeping, and to ensure completeness of results. Beginning September 2, 2025, results for the ‘Resistance Analysis of HIV-1 Protease and Reverse Transcriptase’ (Protease-RT) and ‘HIV-1 Integrase Resistance Genotype’ tests will be combined into a single ‘HIV-1 Resistance Genotype Report’.
